This man had not, however, been elected into the premiership, and consequently never earned his own governing mandate. He had succeeded into the job when his predecessor was forced to resign under internal pressure. While the coup was not entirely of his own orchestration, this prime minister had played an important albeit implicit role in facilitating it.

His predecessor is an altogether different story. Initially elected with much fanfare by a buoyant nation on a platform of hope and change after years of rule by an authoritarian and right-of-centre leadership, his premiership had by its tail-end been reduced to a lethargic disappointment.

Though heavily criticised and the subject of mass ridicule, he was still able to pull through with a mediocre win at the general election. Remaining defiant, he announced that he would fulfil the mandate of the people and see through his term as prime minister.

However, his own party members had other plans. Disenchanted with his leadership, the internal politicking began. New pacts were formed, loyalties shifted and deals were cut. Attention now turned to our protagonist, the then-deputy prime minister, a man long known for his loyalty and methodical methods.

Convinced that the gamble would pay off, he began to make his moves. Though his actions were not overt, the die was cast and the message was clear. In the end, faced with open revolt and under immense pressure, the incumbent prime minister had no choice but to announce that he would step aside within a year. Even that was too long and before the year was up, his premiership had crumbled to an abrupt end.

Expectations were high for the new prime minister. In an elaborate campaign to both distinguish himself from his predecessor and spruce up his public image, he took on the mantle of a statesman with vision for leadership and change.

This led to a popularity high and favourable approval ratings. Soon, the opportunity for snap polls presented itself. With the benefits of a honeymoon period still lingering, it would have been the perfect time to call it. Yet an indecisive itch overcame him and he decided against it at the last minute. Instead, he thought it best to wait for a better opportunity.

In hindsight, that was perhaps his biggest mistake.

As days went by, the public began to realise that a man who has been a part of the system for so long is really incapable of change. His promises of a better future began to ring hollow amidst growing economic uncertainty.

The longer he was into his administration, the more problems unravelled. Financial irregularities and scandals involving his party members made dimmer and dimmer the prospect of a desirable moment for a general election.

Yet the prime minister kept waiting and waiting, until by the time he knew it, his government’s term of office was up. In the end, what was supposed to have been snap polls that would have leveraged upon his then-popularity and caught his opponents by surprise had become an exercise that was easily predicted by all and sundry.

Having rigorously prepared for so long, the hungry opposition wasted no time in pouncing aggressively. Without the element of surprise, the prime minister had lost the upper hand and soon found himself on the defensive. Election Day came and the nation delivered its result. Though not very conclusive, it was certainly very clear about one thing: the people had rejected the prime minister.

After a short period of denial and desperate attempts at forming a coalition, the prime minister had little choice but to accept his fate. Indecisiveness, poor decision-making and the inability to carry out necessary reforms are traits that will characterise his legacy as a short-lived prime minister who had never won his own mandate.

Medical Negligence- ChemoTherapeutic Drug Extravasation into Right arm causing burn, gangrene and skin graft.

On the 2nd February 2012, I received a complaint and visited Mdm T, a 60+ yo lady warded in QEH2.

This pleasant but unfortunate lady was diagnosed with BREAST CANCER and underwent a Left sided MASTECTOMY (Operation to remove cancer tissue affecting the breast and surrounding areas).

She was put on a regime consisting of Six cycles of CHEMOTHERAPY. Each Chemo cycle involves the intravenous injection of Chemotherapeutic drugs.  These Chemotherapeutic drugs have a lot of side effects which can include Nausea, vomiting, hair loss, fatigue, infection and bleeding (just to name a few).

Chemotherapy drugs work by damaging the dividing cancer cells and preventing further reproduction. These drugs are toxic and can also damage normal cells as well and so have to be administered safely and with care.

On the 28th December 2011, Mdm T went to QEH2 for her final (Sixth chemotherapy cycle). As usual, an Intravenous needle and IV drip set was set up on her Right arm to deliver the Chemotherapeutic drug, but this time her tissues started to swell up and her skin started to burn, blacken and was associated with much pain. The chemo drug had Extravasated (leaked out) of the vein due to the IV needle wrongly placed and puncturing the already fragile vein.

The IV Chemo drug was stopped but the damage had  already been done by the toxic chemo agent. Mdm T was not happy as she noted that the paramedical staff attending her was chit-chatting, laughing and watching TV, instead of concentrating on what he was doing. Also the ward was full of relatives and visitors although it was not visiting hours.

As a note, The sixth and final Chemo cycle had to be continued despite what had happened.

What followed was almost two months of extra pain and suffering where her Right arm tissue was burnt, swelled up, developed gangrene and had to be debrided (cleaned).  It was also painful to move her right arm and fingers.

On 18th January 2012, Skin Grafting operation was done  where skin from her legs were removed and used to cover her right arm.

All this could have been prevented with a bit of extra care.

It is hoped that the hospital takes this case / incident seriously and do an independent study or audit so as to prevent future medical mistakes or negligence like this from occurring again. There needs to be a tightening up of their SOP (Standard Operating Procedure) and proper training of staff involved in administering of Chemotherapy.

Being the second case of Medical mistake reported to us, it is to be asked if this is just the tip of the iceberg.

How many more unreported Medical negligence, Medical mistakes or even ” Near Misses” are there that is causing preventable, un-necessary pain, suffering, morbidity and (God Forbid) Fatality?

Medical Negligence- Aspiration Pneumonia/ Lung collapse due to Ryle’s tube inserted wrongly into lung.

Today I went to the HDU (High Dependency Unit) of QEH (Queen Elizabeth Hospital), Kota Kinabalu due to a complain I received about a Medical Negligence case, where a 50yo gentleman had to be put on ventilator machine for 4 days due to aspiration caused by his Feeding tube (also called NG (NasoGastric) tube or Ryle’s Tube) which is supposed to go into his stomach was instead wrongly inserted into his Right lung.

This gentleman suffering from Falx Meningioma (brain tumour) had been operated three times already, and has been dependent on Ryle’s tube feeding since last year. The Ryle’s tube has to be changed every two weeks.

He went to OPD Luyang on 28th January to change his Ryle’s tube, which was done by two Male paramedical staff. But although still having coughing his wife who accompanied him was told everything was ok and allowed to go home. The wife although suspecting something was not right took her husband home.

At home the wife fed him liquid (milk) through his ryle’s tube and noted that he was turning blue and had difficulty in breathing.

He was rushed to the Casualty department where it was found out that the ryle’s tube instead of being in his stomach has gone into his right lung causing his whole right lung to collapse. A Chest x-ray also confirmed that the Ryle’s tube was indeed in the Right lung.

Luckily, he was brought in fast and was able to be intubated and put on a ventilator machine for 4 days and his lungs were able to slowly expand again.

This is a clear case of medical shortfall and negligence, that could have easily ended in a fatality.

To err is human and we are not here to put blame or point fingers on the individual but to ensure that the Medical system is tuned or even changed to ensure that  safety and patient and staff care is given its rightful important place.

The relevant authorities need to investigate this case to ensure that such incidents does not happen again as it could have easily have ended in prolonged pain, discomfort and suffering to the patient, or even Death.
